Genesis Lab, LLC Billing and Collection Policy

Genesis Lab is a multi-specialty anatomic pathology laboratory. This billing policy reflects the intention of collecting appropriate and reasonable fees for these services.

Commercial Insurance Carrier

Coinsurance, copayments, and deductible amounts are determined by the insurance carrier. In the event that we do not participate in an insurance network, please contact our billing department.

Secondary Insurance

If there is a secondary insurance that supplements the primary coverage, we will file the claim with the secondary carrier when provided with the necessary information.

Medicare / Tricare / All Governmental Payers

Federally funded programs do not permit Genesis Lab to waive any deductibles, coinsurance, or copays. It is the policy of Genesis Lab to bill any secondary insurance payers for the deductible, coinsurance, and copayment amounts. If after all payers have remitted payment, it is Genesis Lab's policy to bill and collect from the patient any remaining amounts from any deductibles, coinsurance, or copayment as determined by the payer.

General Billing Information

If Genesis Lab does not have the complete or correct billing information, we may send the patients a bill for the charges. If we are provided with complete and accurate insurance information, Genesis Lab will then submit a bill to the insurance carrier. If we have already billed the insurance company and have had no response from them after a reasonable length of time, we may contact the patient to assist us with the insurance company to process the bill and/or appeal the claim. This is to avoid any possible timely filing issues with the insurance carrier. Genesis Lab will work with the patient to establish reasonable payment arrangements. The patient will only be sent to a collection agency if they fail to forward received payments for our services from their insurance company. Sending patients to a collection agency will be our last resort.

Frequently Asked Billing Questions

Why did I receive a bill from Genesis Lab?

In many cases, when a gastroenterologist conducts a procedure such as a colonoscopy or upper endoscopy, they remove a tissue specimen from the patient. A pathologist (physician specializing in tissue diagnosis) is sent these specimens by your gastroenterologist. Using a microscope, the pathologist analyzes your tissue specimen. The pathologist bills your insurance company directly for their services. Therefore, if you receive a bill from Genesis Lab, it is because we were asked to diagnose your tissue specimens by your physician.

I received a bill from Genesis Lab and have questions regarding the tests and their results. What should I do?

Please contact your physician to receive an explanation of the tests ordered and results received.

What should I do with the bill I received from Genesis Lab?

In most cases, if you received a bill from Genesis Lab, you owe a co-pay, deductible or other additional payment required by your insurance company. If you have any questions or concerns about the bill, contacting your insurance provider or referring to your insurance policy is recommended. You may have also received a bill from Genesis Lab because we were either given incorrect insurance information or no insurance information. In this case, you may have received a bill for the full amount of Genesis Lab's services. Please ensure that you always give your physician all the information from your insurance card prior to any procedure or visit.

If Genesis Lab billed your insurance provider initially but did not receive a timely response from the insurance company, Genesis Lab will bill you directly. Please contact Genesis Lab billing department in this event and ensure that we have the correct insurance billing information. If we did not initially have the correct information, we will re-submit the claim with the revised information.
